At the end of the intake form on the bottom of this page, you’ll be asked to check a box to confirm you’ve read and agreed to the waiver.

2. Complete the Intake Form
Please take your time to answer all the questions thoughtfully and with as much context as you feel comfortable sharing. You may take up space with your answers.

3. Schedule Your Intake Video Call
After submitting your form, you’ll be directed to the final step of this intake process, where you can book your intake video call at a time that works for you.
